APPOINTMENTS
 
Initial appointments can be scheduled by calling the office. Please arrive 15 to 20 minutes early to complete the necessary paperwork. 
 
If you prefer, you may download most of the paperwork from the Forms Page and complete it at home and bring it to the first appointment.  They will print best if you customize the printing size to 40%.
 
Expect to spend  45 minutes to an hour at the first visit.  If a patient needs to cancel an appointment, we request they provide 24 hour notice, so other patients might be seen.   
 
OFFICE HOURS
 
Monday, Tuesday & Thursday: 9 AM to 4:30 PM
Friday: 9 AM to 4 PM
 
INSURANCE
 
Most insurance plans are accepted.  Secretarial staff can provide further information.  Your insurance carrier may also be able to confirm our participation in your plan. 
 
PAYMENT POLICY
 
Financial responsibility for services rest with the patient regardless of insurance coverage.  The office will file insurance claims when supplied with the patient's insurance information.  It is the patient's responsibility to provide current information regarding insurance and demographic information.  Patients my be required to make a co-payment with each visit as indicated by any particular insurance policy.Visa and MasterCard are accepted.
 
The office will complete forms required by patients for insurance, return to work, leave of absences, FMLA, disability, etc.; however, if a particular form requires a lengthy amount of time to complete, the patient may be charged a fee for this service.
 
TELEPHONE POLICY
 
A receptionist in our office answers telephones during regular business hours, 9 AM to 5 PM M-Th & 9 AM to 4 PM Friday.  After hours and on weekends, emergency calls are answered by and answering service and the psychiatrist is contacted to return emergency calls.  We request non-emergency calls be made during scheduled office hours.
 
PRESCRIPTION REFILLS
 
To provide the patient with the best possible medication service, we request all prescription refills be called to the office or pharmacy at least 2 days before needing the refill.  Prescriptions that need to be written must be requested 24 hours in advance.  This will give us ample time to have the prescription approved and provide adequate documentation.  Prescription requests must be made during regular office hours, Monday through Friday.
